Elversberg coach Horst Steffen is close to be appointed Werder Bremen boss, the Deichstube portal said on Wednesday.
The fan website has close contacts within the club and is widely viewed as reliable.
Steffen guided tiny Elversberg to third in the second tier with a vibrant brand of attacking football. They narrowly missed out on promotion to the Bundesliga when they conceded in stoppage time in the play-off with Heidenheim on Monday.
Steffen will replace Ole Werner, who parted ways with Bremen after opting not to sign a new deal amid frictions with the board.
The Northerners finished eighth in the Bundesliga season just finished.
Deichstube said 56-year-old Steffen was signing a deal until 2028 and Elversberg would receive six-figure compensation. An announcement could come as early as Thursday.
Germany assistant coach Sandro Wagner is set to fill the vacancy at Augsburg, while Como's Cesc Fàbregas is favourite for the RB Leipzig job.
Promoted Cologne are also looking for a new coach and former Paderborn boss Lukas Kwasniok has moved in to pole position, Sport Bild reported.
He had indicated he wanted a break after leaving Paderborn following an impressive fourth-place finish in the second tier.
But he appears to have had a change of heart after meeting sporting director Thomas Kessler for "intensive talks."
Former Union Berlin coach Urs Fischer had also been in the running, reports said.
