Mine is with Fidelity, I haven't had any problems and I'm pretty sure mine is without annual fees but I have had it for years so the balance is high enough I may be getting around fees that way?
I also rolled my 401k into an IRA after getting laid off, the 401k was with Fidelity so I rolled it over into an IRA with them, that was a pain in the neck but it turned out to be the fault of my former employer not Fidelity. They had been acquired and had done some paperwork incorrectly so Fidelity was required to get approval from an entity(my former company) that no longer existed and that delayed the paperwork until it got straightened out.
